The purpose of this document is to alleviate the manufacture, distributor and retail outlet for "ayahuasca tablets" of any and all liability arising out of any injury caused
as a result of ingesting "Ayahuasca".
1) I am over 18 and an adult under the laws of my State and Country.
2) I have no medical conditions and am not mentally disabled. I am in good health and capable of making my own decisions.
3) I am either:
a: In good health, with no preexisting medical conditions whatsoever & currently under no medication, prescription, over-the-counter or otherwise. or
b: Under the care or advice of a medical professional who has conducted an examination and recommended Ayahuasca as a dietary supplement for me.
4) I clearly understand that all products & information received through "Ecstacy Melrose" are provided "AS IS" and without warranty, express or implied. All implied warranties of merchantability, therapeutic effects, fitness and taking this product for a particular use, effect or purpose are hereby excluded.Acceptance
5) In no event will "Ecstacy Melrose" be liable to me or anyone else for any damages or injury caused by ingestion of this product or by it's misuse or improper use, or action taken by me in reliance on information or views either verbal or in literature purchased through "Ecstacy Melrose" or for any incidental, consequential, special, or similar damages.
6) I agree that Ecstacy Melrose's liability, if any, for damages (including, without limitation, liability arising from physical harm caused by any ingestable products, liability arising out of contract, negligence, strict liability, tort, shall not exceed the cost of the product which I purchased from Ecstacy Melrose.
7) I herein agree to without limitation indemnify and hold harmless Ecstacy Melrose, The Manufacturer Shamanic Transformations and any and all other parties involved including but not limited to their owners, officers, affiliates, associates, & employees for any and all damages incurred as result of ingestion of this product. These parties shall not be liable under any theory whatsoever.
8) In no event shall "Ecstacy Melrose" be liable for any damages other than the amount paid by me for product Any and all other damages, direct or indirect, special, incidental, consequential or punitive, are hereby excluded.
9) I do not intend to use this product for any illegal purposes. I understand that this product is not intended for illegal use or use in conjunction with any illegal substances.
10) I agree not to sell, distribute, give away this product to anyone unless they are over 18 years of age and have read and signed this document.
11) I am not a law enforcement officer, nor am I a government employee. I am not associated with nor affiliated with any government agency nor am I ordering product under the instruction of any government agent or agency.
I have clearly read and understand any and all Cautions & warnings, including but not limited to the warnings printed on the label (if any) and any additional Warnings & Cautions. I herein agree to carefully abide by these cautions & warnings. If I am uncertain if this product is right for me or of any of the warnings or cautions I shall consult my doctor prior to taking it.
